Inpatient Rehabilitation is a referral-based program where individuals can be admitted from the community (home or retirement home), another hospital or following an inpatient admission at Woodstock Hospital. Referrals to the program can be submitted by any Primary Healthcare Provider (physician or nurse practitioner) or the LHIN Home and Community Care (CCAC).
Inpatient Rehabilitation is a specialized, referral-based program that provides time-limited, intensive rehabilitative care for individuals with complex medical and physical conditions. Individuals can be admitted from the community (home, retirement home), another hospital or following an inpatient admission at Woodstock Hospital, once any acute medical issues have been stabilized.
Inpatient Rehabilitation is focused on an individual’s ability to improve function and independence in their activities of daily living (eating, dressing, walking, toileting) in order to return to a community-based setting (home, retirement home).
With a highly skilled team of medical, nursing and allied health professionals, a coordinated rehabilitative plan of care is developed based on individual and specific care needs. Regular intensive rehabilitative therapy improves independence, mobility and overall functional ability so individuals must be able to actively participate in and tolerate 2-3 hours of therapy per day.
